Children & Youth Health & Power of Attorney Form -- for campers and volunteers under 18 years of age. Bring the form with you to camp--it is no longer necessary to mail a copy to the Conference Office.
Adult Health Form -- for adult volunteers 18 years of age or older

2012 Cabin Assistant Application -- use this form if you have will have completed the 9th grade by summer 2012 and are less than 18 years of age.
2012 Adult Volunteer Application -- use this form if you are 18 years of age or older.
2012 Camp Nurse Volunteer Application -- use this form if you are volunteering to be a camp nurse or doctor.
